Role summary

This is an exciting opportunity for a motivated and innovative individual with Cardiology experience to join the Heart Failure team as a Band 6 Clinical Nurse Specialist. The role involves providing specialist heart failure nursing treatment and care within the hospital and community, developing autonomous clinical decision-making skills, and contributing to a multidisciplinary team. The team focuses on personalized, evidence-based care to prevent admissions, reduce length of stay, and provide education to patients and families.

Essential criteria

  • Registered nurse on relevant part of the NMC register
  • Degree qualification or equivalent 120 credits at L6
  • Post registration qualification within area of speciality or equivalent experience
  • Completion of all Band 5 (preceptorship) competencies
  • Provide evidence of supporting/ managing a range of learners in practice
  • Provide evidence of supporting colleagues to promote effective learning in practice
  • Extensive post registration experience within specialist area.
  • Knowledge of current issues within the acute health service.

Desirable criteria

  • Registered on MSc Pathway - completion of HTPA Competencies
  • Act in the role of Practice Assessor or supporting the progression of learners / supporting challenges as needed
  • Counselling qualification
  • Palliative care course

Main duties of the job

This is an exciting opportunity to join our team as a Band 6 Heart Failure Clinical Nurse Specialist. This is a fixed term contract to cover secondment.

We are looking to recruit a motivated and innovative individual with experience in Cardiology to be part of a team that provides specialist heart failure nursing treatment and care within UHS and as part of the commissioned community service.

The role provides an opportunity to learn about heart failure as a specialism and develop as an autonomous clinical decision maker with the support of a multidisciplinary team.

The UHS Heart Failure Team is a specialist multidisciplinary team that provides care and management of patients with heart failure within UHS as well as in the community in Southampton and South West Hampshire. The nurses are community or hospital based and have the opportunity to rotate between the two areas.

The post holder will be part of a multidisciplinary team including specialist nurses, consultant cardiologists, a pharmacist and administrative support staff.
