Role summary
This role is for a Band 5 (developmental) or Band 6 Community Mental Health Nurse within the Ferndown and West Moors Older Persons Community Mental Health Team. The post holder will provide assessment, treatment, and support for older adults with organic and functional mental health illness. Key responsibilities include acting as a dedicated Care Coordinator, managing a caseload, and providing person-centred, holistic care to aid patient wellbeing and recovery. The team offers comprehensive supervision and peer support.
Key responsibilities
- Act as dedicated Care Coordinator for a caseload of patients.
- Provide person-centred, holistic care to assist in aiding patient wellbeing and recovery.
- Work as part of an established integrated multi-disciplinary team.
- Prioritise caseload and balance service demands.
Essential criteria
- Registered Mental Nurse
- Valid registration with NMC
- Evidence of continuous professional development
- Valid RMN registration
- Job Specific Experience
- Able to assess, plan, implement and evaluate programmes of care.
- Risk assessment and management
- The ability to prioritise and meet deadlines; organise work of self and others; supervising, teaching and assessing; self-motivated with the ability to motivate others; effective resource management.
- Skills in managing complex cases.
- Works well in a multi-disciplinary team.
- 2 years experience post qualification
Desirable criteria
- Uses initiative and can make decisions. Manages difficult situations effectively.
- Awareness of Primary Care and Steps to Wellbeing.
- Applies current research and theory into practice.
- Information Technology
- Competent computer skills, experience inputting information on RIO
- Additional requirements
- Demonstrable skills in written and spoken English, adequate to enable the post holder to carry out the role effectively

Job summary
Dorset HealthCare University NHS Foundation Trust are seeking to recruit a Band 5 (developmental) or Band 6 Community Mental Health Nurse to work our well-established Ferndown and West Moors Older Persons Community Mental Health Team.
If you are a RMN with experience working with people suffering severe mental illness whether this be inpatient, or community based we would love to hear from you.
This post is 30 hours per week (non working day must be a Tuesday or Thursday). This post is being extended to allow for applications from newly qualified nurses, or those practitioners currently working at a Band 5 and looking to develop with support towards a Band 6 whilst working in the community.
The Service is a specialist team providing assessment, treatment and support for individuals suffering from organic and functional mental health illness. Experience of working with older adults with organic/functional mental illness in an inpatient or community setting would be an advantage.
