Role summary
This role is for an experienced Band 3 Healthcare Assistant to join a community heart failure service. You will provide support and healthcare to patients and carers in their homes or clinic settings, under the direction of a Community Heart Failure Nurse Specialist. Key tasks include assisting with assessments, monitoring vital signs, venepuncture, and reporting patient status changes. The position offers ongoing training and development, with potential to progress to a nursing associate role. A full UK driving licence and access to a car are essential due to the community-based nature of the role.
Key responsibilities
- Support the Heart Failure Team to deliver against key performance indicators and input data for clinical outcome measures.
- Assist Community Heart Failure Specialist Nurses in care planning, assessment, monitoring, and review of patients.
- Independently visit patients at home to carry out basic heart failure assessments and clinical tasks like vital sign monitoring, venepuncture, and skin care inspection.
- Support the management of patients in and out of the Heart Failure Virtual Hospital.
- Work closely with GPs, integrated community teams, and social care partners.
Essential criteria
- Higher Professional Diploma in Health and Wellbeing (NVQ/QCF - Level 3) or a recognised equivalent course
- Willingness to undertake further training as appropriate
Desirable criteria
- GCSE grade A-C or equivalent in English and Maths
- Experience of working as a Band 3 HCA in the community or an acute Trust
- Experience of working as part of a team
- Experience with the use of Microsoft Office and a variety of databases
- Experience with the NHS patient information database (Systm 1)
- Experience of nursing patients with Heart Failure or cardiac conditions
- Experience with Chronic Disease advice and management
- Experience of nursing patients with Palliative needs
- Competent in the use of manual blood pressure, pulse, oxygen saturation and weight and able to record and interpret the measurements / willingness to undertake
- Venepuncture / willingness to undertake
- Demonstrate an acceptable level of numeracy and literacy
- An awareness of risk management.
- Knowledge of multi-agency services available and processes for referral
- Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being
- Excellent communication skills and ability to build effective relationships oAbility to analyse, interpret and record data in a clear, and succinct manner oAbility to work under pressure, prioritise effectively and manage competing demands.
- Ability to work with minimal supervision

- Flexible about work location -The post holder will be required to travel around all 4 localities in West Hertfordshire
- As this is a community-based role, with travel between patient homes, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
