Role summary
Join an innovative, caring, and agile Community Nursing team in Welwyn and Hatfield, part of an award-winning service with a 'Good' CQC rating. This role involves delivering high-quality nursing care in patients' homes to prevent hospital admissions, facilitate discharge, and manage long-term conditions. The successful candidate will be supported to develop skills within a dynamic and professional team committed to patient care and trust values. Community nursing experience is preferred, but training and development opportunities are available.
