Role summary
This role is for an experienced registered nurse to join the District Nursing Service in Conwy and Denbighshire. The successful candidate will provide clinically effective care within a team, focusing on maintaining patients in their homes and preventing hospital admissions. There are opportunities for development towards Band 6 District Nurse or Nurse Practitioner roles. The job requires excellent communication skills and the ability to travel between patient homes.
Key responsibilities
- Provide clinically effective care within the District Nurse Team.
- Deliver high quality evidence based care in an integrated way with partner organisations.
- Maintain patients in their own homes and communities, averting hospital admissions.
- Empower patients towards self-management and implement seamless patient-centred care.
- Travel between patients' homes both within towns and rural areas in a timely manner.
Essential criteria
- Registered nurse or within 2 months of gaining PIN.
- Knowledge of Primary Care.
- Effective interpersonal and communication skills.
- Ability to maintain high standards of care
- Clinical competency
- Evidence of ongoing professional training.
- Knowledge of Professional codes of conduct.
- Broad knowledge of relevant community nursing skills and competencies.
- Working unsociable hours Ability to travel between patient's homes in a timely manner.
