Role summary
This is an exciting opportunity for a Level 1 Band 5 registered Social Worker to join the East Staffs Community Hospital Team. The role involves working closely with a multi-disciplinary team to support the delivery of personalisation and social inclusion, in line with the Care Act 2014. The post holder will address care/support issues, promote independence, prevent hospital admissions, and improve health and wellbeing. Key responsibilities
- Support the delivery of the personalisation and social inclusion agenda, in line with the principles of the Care Act 2014.
- Work closely with multi-disciplinary teams/agencies to address care/support issues.
- Promote independence and prevent unnecessary hospital admissions.
- Produce a high standard of recording Statutory assessments and other documentation that support discharge pathways.
- Work within principles of Recovery and Personalisation, empowering individuals through strengths and asset-based models of assessment and intervention.
- Take responsibility for your own Continuous Professional Development.
Essential criteria
- Professional Social Work qualification, DipSW, CQSW, CSS, BA, MA or Foreign qualification where certified by the Professional Social Work regulatory body
- Ensuring current registration with the Professional Social Work regulatory body at all times
- A commitment to on-going personal and professional development
- To undergo appropriate training to achieve Assessed Year in Practice (ASYE)
- Good keyboard / IT skills including word processing, spreadsheets, email and PowerPoint
- A clear understanding of and commitment to equal opportunities and anti-discriminatory practice
- Experience of working with Adults and older Adults with mental health needs and their carers
Desirable criteria
- Experience of social work in other user groups/settings i.e. Children and Families, Learning Disability, Mental Health.

An exciting opportunity has arisen within East Staffs Community Hospitals for a Level 1 Band 5 registered Social Worker.
You will be part of the East Staffs Community Hospital Team and will work closely within the multi-disciplinary team members including Discharge Liaison Nurses, Discharge facilitators, Therapists, Social Workers Social Care Assessors, Nurses, Partner Agencies and Community D2A Teams.
