Role summary
This role involves working as part of a multi-disciplinary team within the Stay Well @ Home Team at Royal Glamorgan Hospital. Key duties include undertaking clinical/therapy interventions, administrative tasks, and liaising with community services to support timely patient discharges from A&E and the Acute Medical Unit. The Assistant Therapy Practitioner will provide high-quality care to reduce length of stay, prevent admissions, and reduce readmissions, with opportunities for community visits.
Essential criteria
- Evidence of good education, to include English, Mathematics GCSE or posses equivalent qualification, or skills knowledge and eperience
- OCN/Agored accredited modules or relevant NVQ level 3
- Knowledge relating to a range of core OT/PT activities
- Knowledge of the discharge planning processes, specific conditions and forms of therapy
Desirable criteria
- Expected to achieve competency based training within a two year period
- Administrative skills
- Experience of team working
- Experience of working in a medical or rehabilitation setting
- Excellent verbal and written communication skills
- Numeric skills
- Computer literacy and keyboard skills
- The ability to work independently whilst under appropriate supervision
- Welsh Language Speaking & Listening Skills (Level 3 and above/B2)
- Ability to work with patients with communication barriers/motivational issues
- Ability to work independently and as part of a team
- Ability to motivate self and others
- Confident in approaching tasks

Job summary
An exciting opportunity has arisen to join the NHS Wales award winning Stay Well @ Home Team at the Royal Glamorgan hospital. We are looking for a skilled, organised and enthusiastic individual to join our established and friendly, integrated team of Occupational Therapists, Physiotherapists, Social Workers and Assistant Therapy Practitioners (ATP's).
Main duties of the job
Key aspects of the role include working as part of a multi-disciplinary team to undertake specific clinical/therapy based interventions, administrative tasks and liaison with local authority and third sector based community services to promote and support timely discharges from the Accident and Emergency (A&E) and the Acute Medical Unit as appropriately delegated by the registered clinicians.
As part of the team, the ATP plays a vital role in providing high quality care and interventions for our patients with the service aimed at reducing length of stay, improving admission avoidance and reducing avoidable readmissions through partnership working with community based services.
