Role summary
This Senior Care Coordinator role is within the Learning Disability and/or Autism Spectrum Condition Keyworker Service. You will coordinate the Dynamic Support Register for children with learning disabilities and/or autism who have mental health needs or present with behaviours of concern. The aim is to prevent admissions to inpatient services and support young people in the community, working across multiple London boroughs. The role involves intensive administration support and chairing meetings to prevent placement breakdown and improve care planning.
Essential criteria
- oRegistration as a Social Worker, Learning Disability Nurse or Registered Mental Health Nurse/or a relevant professional qualification.
- oEvidence of continued professional development.
- oAbility to co-ordinate delivery of regular and timely CETRs for children and young people, with the aim of reducing admissions and improving care planning that supports individuals effectively in the community.
- oAbility to work with operational professionals to deliver robust multi-disciplinary and multi-agency care management.
- oA sound understanding and insight into care management practice, commissioning and funding arrangements for children's health and care within the statutory sector
- oAbility to oversee complex support plans and risk assessments, assess need and risk, including the impact of neurodevelopmental differences or disabilities, behaviours of concern and mental health on day-to-day functioning.

1\. Job Summary
You will be a Senior Care Coordinator within the Learning Disability and/or Autism Spectrum Condition Keyworker Service for Barnet, Enfield and Haringey: providing coordination of the Dynamic Support Register for children with learning disabilities and/or autism with mental health needs and/or presenting with behaviours of concern.
The LD/ASC Senior Care Coordinator provides intensive and highly skilled administration support to the systems around children and young people who are part of the LD & ASC cohort. This includes chairing of meetings that identify strategies and provision that prevent breakdown of the child's current placement and a deterioration in their presentation. The aim is to keep children, young people in the community, to prevent avoidable admissions A & E, Tier 4 inpatient mental health services and 52-week residential placements, and to support young people in this cohort, to be discharged from Tier 4 services and be supported back into the community.
The service is working across Barnet, Enfield, Haringey, Camden and Islington, with work allocated between two Care Coordinators. The two care Coordinators will come together to ensure both locality teams work together to develop shared processes and protocols as well as reporting systems.
