Role summary
This specialist role focuses on advancing community rehabilitation for individuals with psychosis, particularly those referred to inpatient rehab units. The post holder will act as the Key Health Care Professional (KHCP) for Haringey service users, supporting them during HD unit admissions and throughout their 18-month community rehabilitation program. The role involves leading monitoring, consultation processes, and baseline assessments, working within a diverse multidisciplinary team to improve patient outcomes and reduce out-of-area placements.
Essential criteria
- Registered Social worker Recognised training in teaching/mentorship/su pervision or equivalent teaching and assessing qualification
Desirable criteria
- Relevant qualification in Psychosocial Interventions with people with severe and enduring mental health problems. Evidence of other further post- registration education and training
- EXPERIENCE AND KNOWLEDGE
- Significant clinical experience in the community. Significant years post- registration experience, at least one of which will be working with people with severe and complex mental health problems Demonstrable experience and knowledge of exercising clinical responsibility for patients' care and treatment, both as a professionally qualified practitioner and also within the context of a multidisciplinary care plan and under supervision
- Experience of working with people with mental health problems in a therapeutic capacity Demonstrable experience at Band 6 or equivalent. Experience of developing, leading and maintaining effective working relationships with external partners to include Primary Care, Social Care and the Third sector.
- Experience of teaching, training and/or mentoring Experience of working within a multicultural framework
- Experience and knowledge of group processes and facilitating groups
- SKILLS AND ABILITIES
- Understanding of statutory policy relating to mental health care delivery including Care Programme Approach and Dialogue +, National Service Framework for Mental Health, Care Act and the Mental Health Act and Nice Guideline-Rehabilitation for Adult with Complex Psychosis The ability to implement clinical and line management supervision.
- Ability to manage own stress and work in a stressful and unpredictable environment Ability to work under pressure in acute situations with minimal supervision. Ability to communicate effectively within a multidisciplinary team Ability to communicate effectively, orally and in writing, highly technical, and clinically sensitive information to patients, their families, carers and other professional colleagues within a dynamic environment
- Ability to use initiative as appropriate Organisational, assessment and decision-making skills Ability to undertake evidence- based interventions with people experiencing a range of mental health problems. Ability to undertake comprehensive/ complete risk assessment and implement risk management plans

Job summary
This is a fantastic opportunity for an individual seeking to advance in their career into a specialist role in community rehab for the first time, or for someone experienced looking for a change of department.
This role is unique and has been an integral part in development of Haringey's inpatient rehab to community rehab pathway which had great success in reducing out of area rehab placements and reducing the admission rates of service users on the community rehab pathway.
We are a diverse team who strive for further improvements and will be able to support you to develop and enhance your skills and knowledge in this role.
You will be the Key Health Care Professional (KHCP) for Haringey service users with a primary diagnosis of psychosis that have been referred to a inpatient rehab unit
You will be part of the consultation process to support the decision-making process for service users identified for HD unit admissions.
You will be part of an inpatient and community MDT and in co-production with service users, will support and monitor the service user during their HD admission. You will also be the KHCP while the service user continues their 18-month community rehabilitation program when discharge back into the community.
Main duties of the job
To lead on monitoring of service users while they are in a HDU and attend Haringey ICB panels
