Role summary
This role involves supporting individuals experiencing homelessness, sofa surfing, or living in temporary accommodation, helping them re-engage with healthcare services. Key responsibilities
- Carry out outreach plans as prescribed by registered therapists, using autonomy and decision-making within established protocols.
- Provide assistance to clinical staff in the assessment and treatment of patients on discharge from hospital.
- Follow prescribed care plans to support service users at home, including access to GP surgeries, outpatient appointments, and general healthcare.
- Participate in and support the promotion of healthy and independent living practices to patients.
- Work collaboratively with other organisations across health and social care sectors, such as St Mungo's.
- Perform general administrative and non-clinical tasks to support team functioning, including photocopying, filing, liaison, and telephone calls.
Essential criteria
- Maths and English GCSE or equivalent
- Ability to work without direct supervision
- Strong communication skills - electronically, written, verbal
- Experience of working with adults in a health and social care setting
- IT skills (emails, word docs, excel)
- Ability to prioritise tasks
Desirable criteria
- Previous experience as a support worker
- NVQ level 3 in direct care
