Role summary
This role is for a Band 6 Nurse within the Haltemprice CMHT, supporting working-age adults with mental health needs. The successful candidate will manage a caseload of community adult clients with complex needs, applying evidence-based principles to assess, plan, implement, and evaluate interventions. The position also involves providing clinical and supervisory leadership to junior staff and contributing to the day-to-day planning and delivery of the Community Nursing service. Key responsibilities
- To manage a caseload of community adult clients with multiple and complex needs, using evidence based/client centred principles to assess, plan, implement and evaluate interventions.
- To support the team/clinical leads in providing clinical and supervisory leadership to Community Nurses, Nursing Associates, support workers and students within own area of responsibility, within the multidisciplinary team.
- To be jointly responsible with other senior colleagues for the day-to-day planning and delivery of the Community Nursing service.
- To support the team leads and other colleagues with the implementation of policy, protocol, service/practice development and change within own area of responsibility.
- To continue to demonstrate specialist clinical skills in practice and provide support/advice re service development and the delivery of care, in partnership with service users, carers, staff, managers and other agencies.
Essential criteria
- oPost registration/graduate diploma or equivalent experience in related work area, to demonstrate advanced level of practice.
- oFull understanding/application of relevant clinical practice/standards/audit within identified clinical area.
- oEvidence of promoting/supporting active user/carer involvement/participation
- oEvidence of continuing professional development (CPD) relevant to the clinical area at specialist level of care
- oAble to demonstrate effective communication skills
- oGood time management skills
Desirable criteria
- oEvidence of project work that has impacted on practice and demonstrates a higher level of communication/planning/change that impacts within own area of work/responsibility.
- oLeadership training/qualification/experience
- oAbility to effectively Chair meetings.
- oDemonstrable experience of working in the specific field where the post is held.
- oAbility to lead and motivate staff to embrace change.
- oWorking knowledge of Trust policies and procedures to effectively lead and manage others
