Role summary
This is an exciting opportunity for a passionate Community Nurse to join the Bollington, Disley and Poynton Team. The role involves delivering high-standard nursing care, assessing patient needs, and implementing care plans in patients' homes or community facilities. The successful candidate will work collaboratively with a multidisciplinary team to provide safe, coordinated, and person-centred care, supporting service improvement and making a positive difference to patients and families.
Essential criteria
- Registered Nurse to degree/diploma level
- Current registration with NMC
- Evidence of recent professional development in an up-to-date portfolio
Desirable criteria
- Non- Medical Prescriber Teaching/Assessing qualification
- Knowledge and Skills
- Able to assess, plan and evaluate patient care, considering the needs and desires of the patients
- Ability to assess clinical situations and act appropriately
- Awareness of effects of ill health and the ageing process.
- Awareness of the principles and practice of SSKIN.
- Basic computer literacy including Knowledge of Microsoft office systems and use of email
- Leg ulcer assessment and management, Experience in palliative care, end of life care
- Diabetes care/management
- Understanding and/or experience of managing patients with LTC needs
- Recent previous experience within a comparable role/transferable skills
- Demonstrable knowledge of assessment and therapeutic interventions in area of practice
- Experience of developing programmes of care for an individual or groups of patients/clients and of providing advice
- Ability to prioritise and organise workload effectively
- Experience of using electronic patient/service user record systems
- Ability to communicate in a professional way with people at all levels of understanding either face to face, on the telephone, e-mail or written communication
- Compassionate - with exceptional interpersonal skills with the ability to communicate effectively with staff, patients, carers and relatives (as appropriate) remaining sensitive and empathetic
- Experience of working within a multi-disciplinary team.
- Experience of mentoring students / supervise other junior staff

- Must be able to undertake all aspects of the role whilst working as a lone worker
- Ability to travel across the county with heavy / bulky equipment and the ability to travel from patient to patient, whilst maintaining a consistent caseload
- Able to organise own workload and deal with unpredictable nature of the role

Job summary
An exciting opportunity has arisen for a passionate Community Nurse to join the Bollington, Disley and Poynton Team as B5 Community Staff Nurse
Our team is based at Priorslegh Medical Centre in Poynton and provides care across the Bollington, Disley and Poynton area, supporting a local population of approximately 33,000 residents. We are a committed, friendly and forward-thinking multidisciplinary team with a shared vision to deliver the right care, at the right time, for the first time, helping to improve the lives of our local residents.
We are looking for a motivated nurse with a positive and proactive approach who can work collaboratively with colleagues across community nursing, therapy services and primary care. The successful candidate will work closely with the therapy team and GPs from theMiddlewood Partnership to support safe, coordinated and person-centred care.
This is an excellent opportunity for an experienced community nurse who is keen to further develop clinical skills, support service improvement and make a positive difference to patients, families and colleagues within the Bollington, Disley and Poynton care community.
