Role summary
We are seeking a dedicated Assistant Practitioner to join our dynamic Community Stroke Team. This role involves delivering specialist rehabilitation across Occupational Therapy, Speech and Language Therapy, and Physiotherapy, working with patients individually and as part of a multidisciplinary team. You will deliver treatment programmes and act as a key worker for allocated patients. A full driving licence and access to a vehicle are essential for community visits.
