Role summary
This role involves providing high-quality specialist anticoagulation care, including daily coordination of clinics, dose adjustments, and patient counselling. The specialist will interpret results, manage patient caseloads, and act as an expert resource for healthcare professionals, patients, and carers. It also includes coordinating the home treatment pathway for deep vein thrombosis. The position requires independent clinical judgment and effective communication within a multidisciplinary team.
Key responsibilities
- Run daily complex case anticoagulation clinics and provide Warfarin dose adjustment letters.
- Co-ordinate the management of Deep Vein Thrombosis home treatment pathway, including assessing, ordering investigations, diagnosing, and organizing treatment.
- Manage and ensure the daily provision of postal Warfarin clinics, including telephoning patients and seeking consultant advice.
- Communicate and liaise with the multidisciplinary team to promote effective and quality anticoagulant care.
- Provide support and education for patients and carers regarding anticoagulation therapy.
Essential criteria
- Degree/Diploma in Nursing/ equivalent nursing qualification
- Post graduate qualification or equivalent experience
- Experience of multidisciplinary professional working
- Experience managing own patient caseload
- Teaching/mentoring experience
- Minimum of 2 years experience in an acute setting
- Experience of complex case management
- Knowledge of hospital systems
- Knowledge of vulnerable adult protection issues
- Understanding of current Anticoagulation developments and guidance both nationally and locally
- Excellent communication skills with all members of the multidisciplinary team, colleagues, patients and carers using tact, diplomacy and persuasion as well as authority
- Able to prioritise own workload and issues and that of others where necessary to maximise resources.
- Keen to develop knowledge base
- Able to identify personal skill / knowledge limitations for appropriate multidisciplinary / consultant referral
- Good IT skills with ability to use MS Word and databases
- Good record keeping and written presentation skills
- Ability to remain calm and work effectively in pressure situations

- Anticoagulation course
- Experience of nursing in a specialist field
- Evidence of participation in audit / research
- Awareness and respect for colleagues, patients and relatives cultural, religious and emotional needs and beliefs
- Be prepared to work across sites as and when required
