Role summary
This is an exciting opportunity for an experienced and highly motivated clinician to join the Specialist Palliative Care Service in a Band 7 to 8a progression post. The role focuses on preventing avoidable hospital admissions by delivering proactive, responsive, and highly skilled specialist palliative care assessments within the hospital and community. The post holder will identify patients at risk of deterioration, intervene early, and coordinate care to manage complex symptoms and psychosocial needs. This aims to enable patients to reduce their length of stay in hospital or remain safely in their own homes.
Essential criteria
- Registered Nurse or AHP Non-Medical
- Prescriber / V300
- Mentoring and Teaching Qualification or equivalent
- First Level Degree
Desirable criteria
- Leadership Management Qualification Level 7
- Module in Transforming Integrated Palliative & End of Life Care or equivalent Clinical skills module or equivalent
- MSc in Integrated palliative and end of life care or equivalent, or willingness to undertake/working towards
- Post registration experience working in Specialist palliative Care acute and/or community
- Evidence of on-going relevant professional development
- Experience of working within a multi-disciplinary workforce
- Change management experience Involvement in service improvement and the implementation of change
- Experience of leading on quality improvements
- Experience of leading and managing a team
- Skills and Abilities
- Demonstrates specialist clinical expertise. Ability to make complex clinical decisions.
- Ability to motivate others Analytical skills Decision making skills
- Able to prioritise Able to work under pressure
- Able to use technology to find and appraise evidence Enquiry/problem solving skills Excellent IT skills and use of Microsoft products
- Presentation and public speaking experience.

Senior Clinical Practitioner- Hospital/ Community Specialist Palliative Care. Community Services Division
We are pleased to offer and exciting opportunity for an experienced and highly motivated clinician to join our Specialist Palliative Care Service in a Band 7 to Band 8a progression post (appointment to Band 8A will be considered for applicant who meet the essential criteria with the appropriate experience) . This role has a clear and sustained focus on preventing avoidable hospital admissions by delivering proactive, responsive, and highly skilled specialist palliative care assessments within the hospital and community, enabling patients to reduce their length of stay in hospital or to remain safely in their own homes for as long as possible.
Main duties of the job
The post holder will be appointed at Band 7 initially, with progression to Band 8a subject to the achievement of defined competencies and service expectations (Band 8a appointment may be considered) . The role combines elements of advanced practice with leadership and service development, with a strong emphasis on early discharge, admission avoidance and reducing reliance on acute hospital services.
