Role summary
This role involves working as a SEND Assistant Practitioner / Nurse Associate within a pro-active skill mix team, delivering high-quality services to children and young people with learning difficulties and complex medical needs. You will provide clinical care within school environments, working three days a week during term time. The position focuses on promoting and maintaining the health and wellbeing of children and young people, supporting the Healthy Child Programme, and collaborating with partner agencies.
Key responsibilities
- Care for children with urinary catheters, enteral feeds, medication requirements, and awareness of tracheostomy and stoma care in school and community settings.
- Respond to emergency situations and contribute to specialist individual care plans.
- Provide staff training related to children's care needs.
- Undertake holistic assessments of children, young people, and families, delivering the Healthy Child Programme.
- Safeguard and support the health needs of vulnerable children/young people within the caseload.
- Promote and improve health and well-being of school-aged children and young people to reduce health inequalities.
Essential criteria
- Assessing, planning and implementing individualised programmes of care
- Experience of previous work in health care
- Experience in assessing for and ordering community equipment
- Registered Nurse Associate and on the NMC register or
- Assistant Practitioner qualification
- Excellent communication, organisational and interpersonal skills
- Ability to motivate, negotiate with and influence others
- Excellent Communication Skills
- Understanding of skills required for team working
- Assessing, planning, implementing and evaluating individualised complex activities and programmes of care
- Ability to travel, for work purposes, within the Mersey Care area
- Understanding local and national influences on health care delivery, including Public Health
- Clinical Governance and Risk Management
- Actively involved in professional development
- Knowledge of Health Promotion strategies
Desirable criteria
- oExperience of working with CYP who have tracheostomies, urinary catheters, stomas, enteral feeds and use of associated equipment
- Work based experience

The successful candidate will work 3 days per week during term time only. The hours of work are 8.30am - 4.30pm, Monday - Friday.
The Children's service provides support to children, young people and their families within a progressive universal model of service delivery dependent on assessed need.
The service offers every child/family a programme of screening, developmental reviews and information and guidance to support parenting and healthy choices as defined by current School Health drivers.
