Role summary
This role is for a Community Healthcare Assistant working 25 hours per week, 5 shifts a week, as part of an evening team (18:30-23:30) based in Kidderminster. You will visit patients in their homes across Worcestershire to support them in remaining at home and preventing hospital admissions or facilitating early discharge. The role involves providing personal care, mobility support, simple wound care, and catheter care, working independently and as part of a team with senior nurse support. All hours worked are unsocial, offering pay enhancements.
Key responsibilities
- Visit patients in their own homes, primarily across the South of the county, to provide personal care, mobility support, simple wound care, and catheter care.
- Support patients in remaining at home and preventing admission to hospital, and assist with early discharge from hospital.
- Work independently and as part of a team, with some visits requiring two members of staff, and be supported by a senior nurse on triage.
- Support nursing staff and deliver independently a high standard of nursing care, ensuring privacy, dignity, diversity, and cultural respect.
- Participate in staff meetings and training & development activities to keep skills and knowledge up to date.
- Assist the team to develop and achieve team and organizational objectives.
Essential criteria
- Experience of working with older adults in a health/social care setting
Desirable criteria
- Community experience of older adults with physical and/or mental health problems.
- Experience of working in a rehabilitation or general medical setting.
- Experience of medication administration.
- Skills & Abilities
- Role modelling good behaviour.
- A positive attitude and able to act on feedback.
- Ability to communicate clearly and effectively in English in both verbal and written formats.
- Ability to use IT confidently and competently in daily work.
- Good numeracy skills.
- Aptitude for promoting independence.
- Ability to cope with difficult/emotionally distressing/ challenging situations.
- Able to work autonomously and as part of a team.
- Good organisational and time management skills.
- Good observational skills.
- Ability to work without direct supervision and able to use own initiative.
- Basic knowledge of physical/ mental health conditions.
- A level of understanding of rehabilitation
- Additional Criteria
- Willing to undertake further relevant training programmes.
- Willingness to work flexibly to meet the needs of the service.
- Ability to meet the travel requirements of the post.
- I understand that the Trust will not routinely provide sponsorship to work for this role, and employment on a time limited visa doesnt guarantee sponsorship at point of expiry.
- Level 3 qualification (e.g. NVQ 3, A-level) in Care or equivalent qualification or experience or Level 2 qualification (e.g. NVQ2, GCSE Grade A-C) in Care and will be expected to undertake the relevant Level 3 qualification on appointment to post
- Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust
- I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.

several satellite bases across the county. This vacancy is for the North of the
County being based at Kidderminster Medical Centre and Princess of Wales
hospital in Bromsgrove. The base for this post is Kidderminster Health Centre.
The Team visits

admission to hospital and also supporting early discharge from hospital.
The post is
all worked as unsocial hours hence picking up pay enhancements for all hours
worked which makes this post more attractive.more money for less hours!!
