Role summary
This role involves providing high-quality, person-centred care to adults with frailty and long-term conditions within primary care and community settings. The Healthcare Assistant will undertake routine health monitoring, clinical observations, and support long-term condition reviews. They will also conduct planned home visits to housebound patients and contribute to early identification of patient deterioration. The role requires working collaboratively within a multidisciplinary team to improve health outcomes and promote independence.
Essential criteria
- NVQ Level 3 in Health and Social Care or equivalent
- Phlebotomy certificate/ Competence in venepuncture/phlebotomy
- Full UK driving licence and access to reliable vehicle.
Desirable criteria
- BLS Training
- Care Certificate
- Training in long-term condition monitoring (e.g. diabetes, respiratory or cardiovascular care).
- Training in dementia awareness or frailty care.
- Experience working as a Healthcare Assistant in a primary care, community or other healthcare setting.
- Experience undertaking routine clinical observations (e.g. blood pressure, pulse, oxygen saturations, temperature, height and weight).
- Experience undertaking venepuncture/phlebotomy.
- Experience supporting patients with long-term conditions.
- Experience undertaking planned home visits or working with housebound or vulnerable patients.
- Experience maintaining accurate clinical records using electronic patient record systems (e.g. EMIS or SystmOne).
- Experience working effectively within a multidisciplinary team.
- Ability to recognise and escalate changes in a patient's condition appropriately.
- Knowledge of safeguarding principles and the ability to recognise and report concerns.
- Experience supporting long-term condition reviews within primary care.
- Experience working with older adults living with frailty, dementia or multiple long-term conditions.
- Knowledge of proactive care, frailty management and hospital avoidance.
- Knowledge of local health, social care and voluntary sector services.
- Experience promoting healthy lifestyles and supporting self-management.
- Experience working within Primary Care Networks or NHS community services.

Job summary
We are seeking a compassionate and motivated Healthcare Assistant to join our Primary Care Network team.Working as part of an integrated multidisciplinary team, the HCA will deliver high-quality, person-centred care to adults living with frailty, multiple long-term conditions and complex health needs across community and primary care settings.
This is an exciting opportunity to play a key role in delivering proactive, person-centred care to some of our most vulnerable patients. Working under the supervision of registered clinicians, you will support routine health monitoring, undertake clinical observations and contribute to the early identification of patient deterioration. You will work closely with GPs, nurses and the wider multidisciplinary team to improve health outcomes, promote independence and reduce health inequalities.
