Role summary

This Occupational Therapist role involves providing skilled assessments, treatment, and advice to adult patients on the Acute Frailty Hub. The post holder will take a leading role in assessment, diagnosis, and treatment goal formulation, as well as advising patients, carers, families, and other agencies. They will also provide daily leadership within the Multi-Disciplinary Team, ensuring a quality occupational therapy service and contributing to timely patient discharge.

Key responsibilities

  • Assess patients for their potential for rehabilitation and suggest therapy goals.
  • Manage a rehabilitation clinical caseload alongside others within the multidisciplinary team.
  • Provide a quality occupational therapy service to patients on the Frailty unit.
  • Ensure a range of therapies are on offer daily to patients including group activities and one to one interventions.
  • Work as an autonomous practitioner, professionally and legally responsible and accountable for all aspects of own work including the management of a caseload.
  • Provide daily clinical leadership and operational management support to the other members of the MDT.

Essential criteria

  • Degree in Occupational Therapy
  • HCPC registration
  • Post graduate experience within a health care setting
  • Post graduate experience of working with complex medical and long term conditions
  • Experience in working within a multidisciplinary team
  • Post graduate experience and knowledge in 3 or more of areas i.e. orthopaedics, amputees, care of the elderly, neurology and Intermediate care.
  • Excellent communication skills - written, verbal and computer skills
  • Able to demonstrate the implementation of the OT process effectively.
  • Ability to manage and prioritise own caseload
  • Ability to work under pressure
  • Ability to delegate tasks appropriately
  • Ability to recognise own abilities and limitations
  • Evidence of recent CPD
  • Ability to travel independently and efficiently across multiple community sites to meet the operational requirements of the role
  • Ability to work flexibly with excellent time management and self-discipline
  • Ability to work autonomously and within a team
  • Ability to translate the national and local agenda to own working and to participate in shaping and implementing the vision for the future development of the Intermediate care services. o
  • Ability to demonstrate a contribution to personal and team continued professional development and a commitment to evidenced based practice.
  • Willingness to undertake the training required to carry out the job
  • Willingness to abide by the Tameside and Glossop Integrated care organisation policies and procedures

JOB SUMMARY:
