Role summary

This role is for a Neighbourhood Case Coordinator within the Hertsmere Proactive Care Service, focusing on community-based, person-centred care. It involves holistic assessment, care coordination, and addressing health inequalities for residents with moderate to severe frailty. The position has both administrative and patient-facing elements, supporting a multidisciplinary team to reduce unplanned hospital admissions. The coordinator will visit patients at home or in clinics to assess and monitor long-term conditions, providing advice and support.

Essential criteria

  • GCSE General Education or equivalent in 5 subjects including Maths and English to a grade C or equivalent.
  • Foundation Degree in Health and Social Care or equivalent qualification or experience.
  • Diploma NVQ Care level 3 or equivalent qualification or experience.

Desirable criteria

  • All new Healthcare Support Workers across the NHS are required to complete the Care Certificate programme.
  • Willingness to undertake further training
  • Experience of working with older people
  • Experience of care giving
  • Experience of working in a health or social care setting
  • Experience with Microsoft office, diary management using outlook or equivalent software.
  • Experience of working in the community setting
  • Experience using electronic clinical records.
  • Skills and Knowledge
  • Excellent verbal and written communication skills
  • Ability to build effective relationships and work as part of a multiprofessional team.
  • Excellent oral and written communication and presentation skills.
  • Proactive and innovative with a basic understanding of community health care services
  • Ability to work under pressure, prioritise effectively and manage competing demands.
  • Self-management skills-managing time and workload
  • Able to follow instructions and work independently without direct supervision
  • Ability to assess a situation, identifying any risk or potential problem and act appropriately
  • Demonstrate an understanding of what is meant by equal opportunities and able to apply knowledge to their practice

  • Be willing to drive across West Hertfordshire, potentially in adverse weather conditions.
  • The applicant would need to be able to use a map and SAT NAV to navigate to patient's homes.

Job summary

We are recruiting passionate and proactive Neighbourhood Case Coordinators to join the Hertsmere Neighbourhood Proactive Care Service. An exciting part time opportunity (26.25 hrs per week) to play a key role in a new team delivering community-based, person-centred care for residents. Focusing on holistic assessment, care coordination, addressing health inequalities and delivering the strategic ambition of a 'neighbourhood health service'. This role has both an administrative and patient facing element.Working as part of a multidisciplinary team supporting a cohort of residents with moderate to severe frailty who have had 2 or more unplanned admissions within the last year, linking them to health, social care and voluntary sector services.

This service will see the shift from reactive care to proactive identification using risk stratification searches within Primary Care records. Searches will identify a cohort of patients who would benefit from targeted intervention to improve outcomes, overall experience and reduce the likelihood of further unplanned admissions.
