Role summary
This role is for an enthusiastic Clinical Lead to join the Chester South Care Community Team. The successful candidate will provide confident support and clinical leadership to clinicians within a multidisciplinary team. Key responsibilities include clinical oversight of a community caseload and ensuring patients receive high-quality, person-centred care. The post is full-time, 37.5 hours per week.
Key responsibilities
- Provide a highly visible clinical profile with leadership and clinical oversight of a designated community caseload.
- Work collaboratively with wider health, social care and third sector partner agencies to meet patient needs.
- Ensure patients receive the right care, by the right person at the right time and place.
- Provide effective, evidence based care to a defined population through case load management and monitoring of care standards.
- Provide clinical leadership.
Essential criteria
- First level registered general adult nurse
- Evidence of teaching and assessing
- SPC qualification in District Nursing or equivalent; demonstrating robust evidence of caseload management, community profiling and application of health needs assessment
- Proficiency in delivering high quality evidenced based care
- To be able to demonstrate continuing professional development
- Ability to work and communicate effectively within a MDT setting
- Significant experience of working within a community healthcare setting
- Experience of making autonomous decisions
- Demonstrate alignment with Trust Values (6 C's: Commitment, Communication, Care, Courage, Compassion, Competency)

Job summary
We are looking to recruit an enthusiastic Clinical Lead to join Chester South Care Community Team. You will be joining a multidisciplinary team including but not limited to; Team Manager, Enhanced Clinical Practitioner, Clinical Leads, Community Nurses, Specialist Palliative Care Nurses, AHP's as well as many other specialist professionals who visit the team.
We are looking for someone who is enthusiastic, self-motivated and who provides confident support and clinical leadership to clinicians alongside working in collaboration with other professionals. Has excellent communication skills and an ability to work with patients and their families/carers and when necessary, act to safeguard individuals who are at risk from harm and abuse.
This post is full time 37.5 hours per week.
