The post holder will be a Rehabilitation Assistant (Band 3), assisting Waltham Forest Falls

Prevention Service in providing a high standard therapy service to patients within the

community.

This is by supporting the multi-professional team in the assessment and implementation of falls

prevention treatment programmes and care plans for patients who may have diverse

presentations and varying physical and psychological conditions.

Agreed treatment programmes / care plans will be carried out by the post holder, at times,

working alone within the patients home.

Main duties of the job

Key Responsibilities:

1. To carry out prescribed treatment programmes and care plans with patients including

those with diverse or complex disabilities and multi pathologies, seeking assistance from

a qualified member of the Waltham Forest Falls Prevention Service as required.

2. To monitor treatment / care plans and evaluate patient progress to a satisfactory

outcome
