Role summary
Knoll Medical Practice is seeking an experienced Clinical Paramedic or Advanced Nurse Practitioner to join their team. This role focuses on assessing and managing patients with acute and undifferentiated conditions, combining face-to-face consultations with daily home visits. The successful candidate will work collaboratively with the GP team and wider multidisciplinary staff, contributing to high-quality patient care. Key responsibilities
- Assess, diagnose and manage patients presenting with acute, urgent and undifferentiated conditions within professional competence.
- Conduct face-to-face and remote consultations, focusing on acute and same-day presentations.
- Undertake daily home visits for housebound and less mobile patients.
- Recognise and manage clinical deterioration, escalating concerns and arranging appropriate onward care.
- Request, review and interpret relevant investigations and act on results within scope of practice.
- Prescribe and review medication where appropriately qualified and authorised.
Essential criteria
- Experience working as a registered Paramedic or Registered Nurse in an urgent care, emergency care, primary care or comparable clinical setting.
- Demonstrable experience assessing, diagnosing and managing patients presenting with acute and undifferentiated medical conditions.
- Experience undertaking comprehensive clinical assessments and developing appropriate management plans.
- Experience recognising the signs of clinical deterioration and escalating care appropriately.
- Experience managing clinical risk and working within professional scope of practice.
- Experience communicating effectively with patients, relatives, carers and multidisciplinary colleagues.
- Experience maintaining accurate and contemporaneous clinical records.
- Experience working autonomously while seeking advice or support when required.
- Experience prioritising and managing a varied clinical workload.
- Experience working collaboratively as part of a multidisciplinary team.
- Experience undertaking or willingness to undertake patient home visits.
- Health and Care Professions Council (HCPC) registration as a Paramedic; or Nursing and Midwifery Council (NMC) registration as a Registered Nurse.
- Relevant clinical experience in urgent care, emergency care, primary care or a comparable clinical environment.
- Evidence of continuing professional development relevant to acute and urgent clinical assessment.
- Appropriate professional indemnity arrangements.
Desirable criteria
- Previous experience working in a general practice or primary care setting.
- Experience managing same-day or urgent care presentations within general practice.
- Experience undertaking home visits and assessing housebound patients.
- Experience managing acute exacerbations of long-term conditions, such as asthma, COPD, diabetes or cardiovascular disease.
- Experience assessing and managing minor illness and minor injuries.
- Experience requesting, interpreting and acting upon clinical investigations, including blood tests and ECGs.
- Experience prescribing or working within a prescribing-led clinical service.
- Experience using EMIS Web or another primary care clinical system.
- Experience working with multidisciplinary teams, community services and secondary care providers.
- Experience contributing to clinical audit, significant event reviews, service development or quality improvement.
- Experience supporting or mentoring students, trainees or other members of the clinical team.
- Experience implementing new clinical pathways, digital systems or service improvements.
- Postgraduate qualification in Advanced Clinical Practice or equivalent advanced clinical training.
- Independent prescribing qualification.
- Relevant minor illness, minor injury or acute assessment qualifications.
- Immediate Life Support or Advanced Life Support certification.
- Additional postgraduate qualifications relevant to primary, urgent or emergency care.

Nurse Practitioner to join our friendly and supportive practice team.
This is an exciting opportunity for a clinician with a
strong interest in acute and same-day care to play a key role in the assessment
