Job summary

The Torbay Adult Health and Social Care Team is recruiting Health and Social Care Co-ordinators to work Monday to Friday between 8am and 6pm, with some weekend and bank holiday working. Hours and working patterns can be discussed at interview.

The role involves acting as a single point of contact for community health and social care referrals, undertaking assessments, applying Care Act eligibility criteria, identifying risks, providing advice and signposting, developing support plans, and acting as a Keyworker.

Applicants should have excellent communication and IT skills, a good understanding of the Care Act and eligibility criteria, and experience working within a health and social care setting. The ability to prioritise workloads and produce clear, accurate records and reports is essential.

Please note:

We welcome applications from candidates who do not yet hold a relevant Health and Social Care qualification. Successful applicants may be appointed to a Band 4 developmental position , with full support to achieve the required qualification and competencies. Once these requirements have been successfully completed, the post holder will progress to a Band 5 role .
