Role summary
This role is for a compassionate and skilled Community Staff Nurse to join the West Kent Urgent Care Service. The nurse will deliver rapid two-hour crisis response to patients in their homes, aiming to prevent hospital admissions and support early discharge. Responsibilities include providing hospital-level treatment in the community, such as IV therapy, end-of-life symptom management, and wound care. The service operates 24/7, 365 days a year, requiring collaboration with health and social care colleagues.
Essential criteria
- You will have a Level 6 Degree or equivalent qualification, or equivalent knowledge acquired through experience plus a diploma in nursing
- You'll need to have recent experience of healthcare gained through education, work experience or previous employment
- You will be a registered health care professional with the Nursing and Midwifery Council
- A full, valid UK driving licence or a full driving licence issued by an EU country
- Knowledge and skills
- You will need to have a customer focussed approach
- Provide patients, clients and their families with the best possible experience
- You will need to be able to be able to use IT systems and databases
- You will understand where to signpost clients and families to alternative services to support behaviour change and improve health
- You will have knowledge of up-to-date clinical practice by using up- to- date skills and adhering to evidence-based procedures and clinical effectiveness frameworks. You will fully understand the role and remit of a health care worker

We're looking for a compassionate, skilled nurse to join our West Kent Urgent Care Service, delivering rapid two-hour crisis response to patients in their own homes. As part of our team, you'll use an extensive clinical skill set to provide hospital-level treatment in the community. This includes IV therapy, end-of-life symptom management via syringe drivers, catheterisation, phlebotomy, wound care, risk assessment and essential nursing interventions. You'll play a vital role in supporting acutely unwell patients who might otherwise require admission, ensuring they receive safe, timely and compassionate care at home.
Working collaboratively with colleagues across health and social care, you'll help deliver a responsive, patient-centred service that makes a real difference to people during moments of crisis.
