Job summary

An exciting opportunity has arisen for highly motivated, passionate clinical practitioner (Registered Nurse or AHP) to be part of a small team working to prevent unnecessary admissions to mental health hospitals of children, young people and adults with a learning disability and/or autism.

As a clinical practitioner you will oversee a cohort of individuals who have been referred to the Dynamic Support Pathway & whom have been identified as having complex needs requiring additional support to coordinate & manage their care.

You will be confident to work autonomously and as a member of a multi-disciplinary team and have the ability to work across numerous agencies whilst keeping the individual at the centre of everything you do.

You will have excellent communication and interpersonal skills, be able to problem solve, prioritise and demonstrate the Trust Values in everything you do.

Whilst the role primarily works with adults, the Dynamic Support Pathway is an all age service and experience of working with children and young people is desirable.

You will join the award winning LDA Collaborative and will be part of making a difference and improving the lives of local people.
