Role summary
This role is for an enthusiastic Band 6 practitioner to join a developing Community Rehabilitation Team in Bradford and Craven. The post holder will proactively engage service users with complex psychosis and Severe Mental Illness (SMI) in their rehabilitation journey. They will work collaboratively with Community Mental Health Teams and partner organisations to coordinate care and ensure appropriate support. The aim is to reduce health inequalities and improve outcomes through recovery-focused rehabilitation services.
Key responsibilities
- Manage a clinical caseload to meet the rehabilitation needs of service users.
- Support patient flow and the development of rehabilitation pathways.
- Implement evidence-based specialist rehabilitation knowledge and skills.
- Be a proactive team member in communication, administration, quality, and service development.
Essential criteria
- Degree or equivalent pre-registration award in Nursing, AHP, Social Work, Psychotherapy
- Short specialist course/s with relevance to specialist clinical area
- State registration with NMC, HCPC, BACP
- Full current UK Driving License
- Professional practice educator training
- Working as an Mental Health Practitioner assessing the needs of service users with severe and enduring mental health problems.
- Mental health rehabilitation
- Clear supervisory experience.
- Professional practice education experience
- Clear understanding or working knowledge of NICE guidance relative to clinical area & quality indicators.
- Experience of using a variety of specialized assessments and models
- Experience of using outcome measures appropriate to the specialist clinical area.
- Able to relate to and empower people to realize and maintain their potential in relation to health and wellbeing.
- Ability to assess function, ability, risk and interdependencies with psychosocial needs contributing to overall assessment of health and well being needs.
- Ability to create, monitor and review treatment plans, taking into account the complex, serious and changing needs of individuals.
- Effectively utilize a wide range of specialist therapeutic skills to provide treatment interventions, both through individual and group media.
- Understanding of the Band 6 role and own developmental needs with regard to the post.
- Understanding of current mental health recovery & rehabilitation agenda and national guidance with regard to same
- Understanding of the role in supporting flow and transition
- Ability to work closely with managers/peers and MDT to deliver high quality service.
- Ability to plan, allocate and supervise the work others.
- To adopt and adapt a professional approach in a variety of situations.
- Ability to utilize information technology
- Ability to use effective and diverse range of communication
- Access to own car, available for work, unless precluded by disability
Desirable criteria
- Professional Practice education accreditation
- Leading an Empowered Organization (LEO) training or equivalent
- Level 2 Chartered Management Institute training or equivalent
- Post registration experience of working with a range of mental health problems i.e. psychosis and personality disorders
- Experience working with individuals having neurodevelopmental disorders
- Experience of working within an inpatient setting.
- Conducting staff reviews
- Knowledge of recovery approach
- Demonstrable innovation in mental health rehabilitation
- Knowledge of relational security
- Experience of service user involvement
- Knowledge of issues relating to substance misuse
- Knowledge pertaining to criminogenic issues
- Experience of project leadership
- Team leadership experience.
