Role summary
This is an exciting opportunity for a Band 6 Occupational Therapist to join the Halton Intermediate Care and Frailty Service. The role involves consolidating clinical knowledge and developing leadership skills within a multidisciplinary team. You will work across various service areas including Single Point of Access, Community Rapid Response, Reablement, and the Intermediate Care Unit. The position requires promoting independence, driving a strength-based approach, and providing therapy interventions in patients' homes or discharge to assess units. You will manage your own caseload, perform patient assessments, develop and evaluate treatment programmes, and contribute to the clinical supervision of students and junior staff.
Key responsibilities
- Hold responsibility for your own caseload.
- Perform patient therapy assessments, provide a diagnosis, develop, implement and evaluate individualised holistic treatment programmes.
- Deliver a high standard of evidence-based care.
- Work in a multi-disciplinary team, follow appropriate pathways of care, liaising with and referring to other agencies as appropriate.
- Contribute to the clinical supervision of students and junior staff.
- Participate in service improvement projects, audits, and in-service training.
Essential criteria
- Under Graduate Degree in Occupational Therapy
- HCPC Registration
- Completion of relevant Post Graduate training
- Previous NHS experience
- Experience of Band 5 /6 core rotations
- Experience of supervising, appraising and training staff and students.
- Experience of managing challenging situations
- Understanding of local and national drivers for service delivery
- Awareness of trust core values
- Continuing commitment to CPD
- IT literate in word processing and spread sheets
- Able to use initiative and be proactive
- Demonstrates being flexible within a team
- Experience of effective contribution to MDT working
- Demonstrate effective communications skills both written and verbal
Desirable criteria
- Post registration qualification in specialist area
- Clinical educators certification
- Band 5/6 experience within Intermediate Care or related community role
- Experience of audits, service development and implementation of change
- Awareness of clinical governance and its impact on therapy services
- Participation in staff management
- Experience of single handed care equipment and approaches
