Role summary
This role is for a Healthcare Assistant within the North Urgent Community Response Team, providing care to patients in their own homes during crises, including end-of-life support. Responsibilities include personal care, wound care, equipment provision, and collaborating with nursing and therapy teams. Key responsibilities
- Provide personal care, wound care, and prescribed equipment to patients in their homes.
- Work collaboratively with the Urgent Community Response nursing and therapy team.
- Contribute to the delivery of care pathways for patients with long-term conditions, palliative care, and rehabilitation needs.
- Work to prevent unnecessary hospital admissions and support early discharges.
- Collaborate with healthcare professionals and agencies to provide integrated services.
Essential criteria
- NVQ Level 3, or equivalent knowledge and experience
- Good general education, including GCSE Grace C in English and Maths
- Experience of working within a health or social care setting
- Able to Communicate effectively using the English language in both verbal and written form
- Good interpersonal skills
- Able to work as part of a team
- Highly motivated
- Able to provide high standards of care

- The post holder will need to be passed fit to perform full duties of the post through occupational health with any reasonable aids and adaptations if necessary. Some roles require additional levels of checks as part of the employment process i.e. roles which come into contact with children, vulnerable adults or their families
- Must hold full and valid driving licence and have access to a vehicle
- Be able to make own travel arrangements to patients, clinics, base and meetings as required
- This is a 7 day a week service and some weekend working will be expected. Hours of service delivery are 8am-8pm 365 days per year

Main duties of the job
This role involves providing personal care, wound care, providing prescribed equipment and working alongside colleagues to respond to service needs. Good communication and collaboration as part of the Urgent Community Response nursing and therapy team is essential. You will have regular supervision and professional development opportunities.
Urgent Community Response is a short term service, we make people safe and refer them on to other appropriate services as needed. Our focus is primarily admission avoidance to secondary care, keeping people safe in their own homes, including people at the end of their life, and also supporting discharges from secondary care or in-patient units.
