Role summary
This Support Worker role involves assisting clients in their own homes with personal care, washing, dressing, and meal preparation, aiming to help them remain at home during intermediate care. The post holder will work autonomously within the Community Crisis Response Team, managing their own priorities and maintaining accurate client records. The role requires independent travel between various community locations and involves supporting clients to achieve their intermediate care goals.
Key responsibilities
- Support clients in their own homes with personal care, washing, dressing, and meal/fluid preparation.
- Work autonomously in the community without direct supervision, managing own work priorities.
- Maintain accurate, comprehensive, and up-to-date client records in line with clinical guidance and LWSW policy.
- Take a holistic approach to client management, carrying out tasks and treatment plans as directed by senior staff.
- Support clients with achieving intermediate care goals within a 6-week period.
- Travel independently and in a timely manner between Livewell Southwest sites and community locations, including delivering equipment.

Job summary
- We currently have a vacancy available for a 28.5 hour Support Worker role. The shifts are variable between 8.00am and 8.00pm as we cover a 24/7 service.
- The Community Crisis Response Team (CCRT) is based at Mount Gould Hospital and is a team of approximately 60 staff members of varying professions
- The role is to support clients in their own homes with providing personal care, support with washing and dressing, meal and fluid prep. Our aim is to enable them to remain in their own homes whilst in receipt of intermediate care in the community and to support them whilst they return to their normal baseline
- The post holder will be required to work alone in various settings in the community without direct supervision
- You must have the ability to frequently travel independently and in a timely manner between Livewell Southwest footprint. Some areas are not served by public transport
- You must be able to travel between clients on our caseload and on occasions, deliver equipment such as mobility aids
- To manage your own work priorities, balancing clinical, professional and organisational demands
