Role summary
This role is for a Community Nurse within the Cambridge East and South City Nursing Service. The successful candidate will provide high-quality nursing care to patients in their own homes or residential settings, focusing on wound care, palliative care, and long-term conditions. The position involves assessing patients, formulating care plans, and working as part of a supportive team to deliver compassionate care. Independent travel across the county with a full UK driving licence and access to a vehicle is essential.
Key responsibilities
- Be professionally and legally accountable for all aspects of your own and delegated work.
- Undertake holistic assessment, including for complex presentations and multi-pathologies, using clinical reasoning skills.
- Use evidence-based practice to ensure clinical care is effective and appropriate.
- Promote independence and avoid hospital admission where possible, ensuring patients receive care in the most appropriate setting.
- Effectively communicate verbally with patients, carers, and colleagues, adapting communication for those with difficulties.
- Ensure accurate and timely written records are kept in compliance with Trust policy and NMC guidance.

- Computer/ IT skills
- Able to assess plan and implement care and formulate care plans.

- Shows a realistic appreciation of the challenges of working with the client group
- Ability to travel independently around Cambridgeshire and Peterborough area meeting required timescales. A Full driving licence and access to a vehicle is essential.
- Flexibility to work over seven days per week
