Role summary
Join Procare Community Services as a Community Matron, delivering proactive, person-centred care to adults with complex long-term conditions and frailty. You will provide advanced clinical assessment, case management, and leadership within an Integrated Neighbourhood Team to support independence and reduce hospital admissions. This role involves coordinating holistic care with various partners and requires an experienced registered nurse with strong clinical decision-making skills. You will manage a caseload of high-risk individuals and work rotationally in the Community Coordination Centre.
Essential criteria
- Registered Level 1 Nurse with current Nursing and Midwifery Council registration
- Qualification or experience equivalent to MSc / level 7, including health assessment module
- Assessor / NMC Supervisor / Mentorship
- Knowledge and Experience
- Advanced clinical assessment and decision-making skills, including the recognition and management of deterioration, frailty, and long-term conditions.
- Experience of managing complex caseloads and coordinating care for patients with multiple long-term conditions and complex health needs.
- Knowledge of safeguarding, risk assessment, health and safety, and clinical governance principles.
