Role summary
This is an exciting opportunity for a Bengali/Sylheti speaker to work as a dementia care advisor/navigator within Tower Hamlets post dementia diagnostics service. The role involves providing post-diagnostic information and support to people with dementia and their families, helping them prepare for the illness progression and access necessary support. You will work with individuals and families, providing information, signposting, and onward referrals to various support services. The position also includes facilitating Bengali Cognitive Stimulation Groups and GRACE interventions, working within a multidisciplinary team, and visiting people in their homes.
Essential criteria
- NVQ Level 3 qualification to diploma equivalent level or above
Desirable criteria
- Qualification in health or social care related subject
- Experience of working with people who have a disability and/or physical/mental health/sensory difficulties
- Experience of using Excel software for data recording and reporting
- Experience of team working
- Experience of co-facilitating and presenting information to groups
- Experience of working with people who have a learning disability, mental health issues and behaviours which may challenge
- Experience of working with people who have communication difficulties or are non-verbal
- Experience of working with paid and unpaid carers
- Experience of working in a community-based health or social care setting
- Experience of gathering health-related data and information
- oExperience of working with people whose first language is not English and of working through interpreters
- Knowledge and Skills
- Understanding of what a learning disability is and the impact it may have upon someone on a day to day basis
- Ability to adapt communication style to communicate effectively with those who have limited verbal communication skills
- Good interpersonal and written communication skills
- Competence in the use of IT software - Microsoft Word, Excel, PowerPoint, Outlook, Teams; Zoom
- To be willing to learn new clinical skills
- Knowledge and understanding of cultural diversity the

Job summary
We have an exciting opportunity for a Bengali/Sylheti speaker to work as a dementia care advisor/navigator within Tower Hamlets post dementia diagnostics service. Consisting of a band 6 post diagnostic Mental Health Nurse and 2 other dementia care advisors/navigators.The aim of the service is to provide post diagnostic information and support to enable people with dementia and their families prepare for what to expect as the illness progresses and get the support they need.The team sits within the broader Integrated Care Mental Health team where patients can be escalated too if they require more intensive support.
Main duties of the job
Work with individuals and their families, to provide information and signposting and onward referrals to support the person to live as best they can with the illness. This may include referrals to the carers centre, Adult Social Care, telecare, among others. We work closely with the Alzheimer Society who run social inclusion groups in the borough. Working with the patient and their carers on a personalised care plan and reviewing before discharging back to the GP.

To offer and facilitate GRACE interventions.
Work with the wider MDT.
Attend Dementia Support Service team meetings.
