Role summary
This is an exciting opportunity for a Band 5 Community Nurse to join the Surrey Downs Health & Care team at East Elmbridge PCN. The role involves assessing care needs, implementing and evaluating holistic care packages, and promoting patient health and wellbeing within the community. You will work as part of a team, contributing to its management and the development of innovative practice. A valid driving licence and access to a vehicle are essential for this role.
Essential criteria
- Registered Adult Nurse with NMC
- Valid Driving Licence (to drive within UK)
Desirable criteria
- V100/Mentorship
- To have an understanding of current issues affecting the NHS
- Understanding of the Clinical Governance agenda
- Relevant post registration experience
- Community experience
- Ability to assess, deliver and evaluate quality care
- Effective communicator

We are looking for nursing staff from all backgrounds including newly qualified, those more experienced, looking for a change, maybe wanting to work locally, excited by the difference of being out within the community with the benefits and challenges that brings.
To work within the team, contributing to team management and the development of innovative practice. To be responsible for the assessment of care needs and the development, implementation and evaluation of holistic care packages.
Please note these roles require post holders to hold a current and valid driving licence enabling them to drive in the UK and have access to a vehicle for work purposes.
