Role summary
This role is for a registered practitioner within the new Health & Justice Service, specifically Reconnect Sussex. The post holder will work autonomously and as part of a team to assess and address the health and social needs of prison leavers from HMP Ford. They will produce holistic needs assessments and risk assessments, making referrals to various services to support health outcomes and reduce reoffending. The role involves co-producing support plans with leavers and partner agencies, and supporting other team members.
Essential criteria
- Registered Mental Health Nurse/ Learning Disability Nurse/Registered General Nurse on NMC Register.
- Professional Social Work qualification, DipSW, CQSW, CSS, BA, MA or Foreign qualification where certified by the Professional Social Work regulatory body. Ensuring current registration with the Professional Social Work regulatory body at all times.
Desirable criteria
- Post-registration experience, evidence of formal courses and training at diploma/degree level.
- Psychological intervention Qualification including EMDR, CBT, CBTp, Family Interventions, CAT, DBT, Structured Clinical Management
- Evidence of extensive post registration experience with working with Service Users experiencing mental health related difficulties
- Experience of working with Service Users who have difficulty in engaging with services and have complex mental health needs
- Experience of assessing, planning, co-ordinating and managing people with health problems in a community setting.
- Experience of engaging with Children and Young People and their families.
- Experience of presenting at local/national level

Main duties of the job
