Job summary
We are currently recruiting for an enthusiastic & motivated Band 5 Physiotherapist to join our Integrated Community Team. We provide a supportive & dynamic service, delivering high-quality, person-centred rehabilitation in patients' homes and community settings.
This is an excellent opportunity for a newly qualified or experienced Physiotherapist who is keen to develop their clinical skills, can demonstrate excellent communication & organisational abilities, and enjoys working as part of a team.
As a Community Physiotherapist, you will work as part of a multidisciplinary team to assess, treat, and support adults with a wide range of health needs. You'll provide assessment & rehabilitation interventions, including mobility assessment, strength and balance rehabilitation, exercise prescription, falls prevention, and functional recovery, helping patients to maximise their independence, and quality of life.
You'll manage a varied caseload with support from experienced colleagues and work closely with health & social care professionals to support people to remain at home, prevent avoidable deterioration, and recover following illness or hospital admission.
