Role summary

This role is for a compassionate Registered Nurse to provide community care to patients in their homes across Taunton, Wellington, and Wiveliscombe. You will manage complex health issues, collaborate with other disciplines, and work closely with the Hospital @ Home and Rapid Response teams. The position involves developing a broad range of nursing skills, making autonomous clinical decisions, and creating holistic care plans, with comprehensive training provided.

Key responsibilities

  • Provide care to patients in their homes, managing a range of complex health issues.
  • Collaborate with various other disciplines and work closely with the Hospital @ Home team and Rapid Response.
  • Develop a broad range of nursing skills, including IV medication administration, palliative and end-of-life care, complex wound management, catheter and continence care, diabetes management, PICC line care, medication administration, and drain tube management.
  • Make autonomous clinical decisions and create holistic care plans with support from colleagues and Team Leaders.

Essential criteria

  • Appropriately registered with the Professional body as per post requirements,
  • Relevant degree or equivalent qualification
  • Evidence of continuing professional development and to have undertaken post registration training
  • Substantial experience in the required setting
  • Demonstrate clinical and managerial leadership ability
  • Experience of working in multi-agency services.
  • Ability to undertake the higher level clinical skills required for the specialist area
  • Experience of mentoring

Desirable criteria

  • Leadership/management programme or prepared to undertake a course
  • Non- Medical Prescriber
  • Coaching and/or mentorship training
  • Substantial experience in the required setting
  • Demonstrate clinical and managerial leadership ability
  • Experience of working in multi-agency services.
  • Ability to undertake the higher level clinical skills required for the specialist area

Our service covers the urban and rural areas of Taunton, Wellington and Wiveliscombe. The usual working patterns are day shifts covering 8am-4pm, 9am-5pm or 10am-6pm. You'll also work one twilight shift per week (2pm-10pm) and weekend cover on a rota which include additional unsocial hours enhanced payments. There are a separate night team. We offer flexible working solutions where possible to ensure a work-life balance.

You'll need to be a car driver with your own vehicle to visit patients.We offer a generous mileage allowance to cover the use of your car for NHS business, which includes both mileage and depreciation.
