Role summary
This role involves working as an Advanced Community Practitioner Occupational Therapist within a multidisciplinary team in Buckingham and Aylesbury. You will provide specialist assessment and treatment, supporting patients at home to prevent hospital admissions and facilitate timely discharges. The position requires clinical and management skills, working closely with various healthcare partners to deliver care as close to home as possible. Occupational therapy is key to rehabilitation and maximising patient independence.
Key responsibilities
- Provide advanced skilled, professional triage and expert assessment of health needs and relevant therapy case management, especially for those with complex needs and at high risk of unplanned admission to hospital.
- Educate and support members of the multi-disciplinary team to intensively case manage these patients.
- Work with a range of professionals across the system to ensure appropriate and timely care, aiming to prevent hospital admission, support early discharge, and reduce GP contact.
- Demonstrate professional skills and competence in leadership, care planning/co-ordination, and care and treatment delivery.
- Provide professional and operational leadership for the local team.
- Engage with RRIC Clinical Leads for continuous service development and deputise for the Clinical Lead as needed.
Essential criteria
- Educated to Degree/ Diploma in relevant profession: Physiotherapy /Occupational Therapy / Paramedic Science / Nursing
- Registered Allied Healthcare Professional with HCPC - Physiotherapist /Occupational Therapist / Paramedic Or Registered Nurse with RCN
- Relevant post graduate clinical training e.g. Advanced History Taking & Assessment
- Recognised teaching or mentorship qualification equivalent to level 3
- Recognised management or leadership qualification
- Relevant post registration NHS / Adult Social Care specialist/clinical experience and competence
- In-depth experience of inter-agency and partnership working
- Experience of working as advanced practitioner/case manager

- Knowledge of managing cognitive impairment and mental well being.
- Supporting self-care, self-management and enabling independence.
- Developing highly specialised programmes of care, providing advice concerning treatment of patients

- Experience of working in a community setting
- Experience participating in research or audit project.
- Highly specialist knowledge in managing patients in the community with complex medical needs, long term/chronic disease management and palliative care
- Highly developed professional knowledge base, supported by educational development and different work environments
- Knowledge of current political drivers, health and social care issues
- Specialist knowledge of governance and risk strategies and their application in a community setting
- Highly developed general management practice, including organisational performance management, finance, human resources and equality and diversity
- Highly specialist clinical and technical skills in managing long term conditions / end of life care
- Strong analytical and judgement skills, with the ability to interpret complex situations and make appropriate decisions
- Excellent interpersonal skills and ability to build and maintain relationships with other health and social care professionals, patients, carers and the public
- Excellent written and verbal communication skills
- Strong IT skills - including Excel and Powerpoint
- Good presentation skills / public speaking
- Ability to plan and manage resources within allocated budget effectively
- Ability to work autonomously without supervision
- Ability to meet deadlines and work under pressure
- Ability to adapt to the demands of a constantly changing environment
- Valid and current UK drivers licence and access to a car with Business insurance for travel throughout Buckinghamshire

Job summary
We are recruiting to both our Buckingham and Aylesbury bases . You will be working with a Multidisciplinary team of Physiotherapists, Occupational Therapists, Intermediate Care Nurses, Community Healthcare Assistant Practitioners and Healthcare Assistants in a clinical and management role providing specialist assessment and treatment, supporting patients at home primarily working on admission prevention but also supporting timely hospital discharge. Our Urgent Community Response service and Frailty Virtual ward team are key partners and we work closely together to ensure care is delivered as close to home as possible.
Occupational therapy plays a key role in the rehabilitation and maximising independence of the wide variety of patients we support on our service.
We work closely with Community nursing teams, Single Point of Access, Primary care and Adult Social Care and there are lots of opportunities for joint working.
As Integrated Neighbourhood teams develop we are committed to RRIC playing an integral part in supporting our local population.
