Role summary
This role is for a Community Support Worker within the Darlington Crisis Team, focusing on Adult Mental Health Services. The successful candidate will provide crisis assessment and enhanced support to adults experiencing mental health crises, aiming to reduce hospital admissions. Key responsibilities
- Support qualified staff with community/hospital visits and provide support/advice over the phone.
- Deliver enhanced crisis support sessions to clients experiencing mental health crises, as care planned by qualified staff.
- Document patient electronic records, including opening referrals and recording contacts to a good standard.
- Liaise with partner agencies and signpost/refer clients to appropriate services under the direction of senior staff.
- Work shift patterns covering a 24/7 service provision.
Essential criteria
- Diploma in Health and Social Care level 2 or equivalent. If applying for trainee post Diploma in Health and Social Care level 2 must be achieved within agreed time
- Care Certificate. If applying for trainee post this must be achieved within 12 weeks of commencement in post
- Key skills in Literacy and Numeracy level 2
- Successful completion of Trust's full HCA Framework within 6-month probationary period. If applying for trainee post successful completion of the Trust's full HCA Framework, including key skills in Literacy and Numeracy level 2, within agreed time
- Previous experience as a Band 2/3 HCA or equivalent in a care environment. (Trainee post: no essential experience required)

- Previous experience of working with people with mental/social health needs or learning disabilities.
- Person centred care
- Awareness of legal frameworks including Mental Health legislation (within agreed timescale)
- Knowledge of Microsoft Office applications (within agreed timescale)

- Competency in record keeping (within agreed timescales)
- Breakaway techniques (within agreed timescale)
- Competency in venepuncture
- Competency in ECGs

The applicant will be calm, consistent approach in challenging clinical situations, be compassionate, creative and flexible in delivering care to our clients and their families/carers. The successful applicant will be given the opportunity to influence and contribute to service development, providing a high degree of job satisfaction and personal progression.
