We're establishing a newIntensive Case Management (ICM) and Rehabilitation Pathway , bringing together specialist professionals to deliver flexible, proactive support for adults with some of the most complex mental health needs.
We're looking for aMental Health Nurses who are committed to building therapeutic relationships, working collaboratively across services and delivering recovery-focused care in the community. As part of this newly established pathway, you'll play an important role in developing a service designed around continuity, assertive outreach and person-centred support.
