Role summary
This is an exciting opportunity for an experienced Band 5 nurse to join a small, supportive, and friendly community team in Neston. The role is fixed-term for 10 months to cover an internal secondment. You will deliver high-quality nursing services and holistic assessments to patients, primarily in their own homes, working alongside various professionals to coordinate care and prevent unnecessary hospital admissions. This position is part-time, 29 hours per week, and requires access to your own vehicle for work.

Main duties of the job
Our eight care community teams are based in various locations within Chester, Ellesmere Port, Neston, Frodsham and the Rural locality covering the geographic area of West Cheshire. It's important that you can drive and have access to your own vehicle for work. But, of course, mileage expenses are reimbursed in addition to your hourly rate.
