Role summary
This is an exciting opportunity for a Registered Nurse to join a successful and established Continuing Healthcare (CHC) team on the Wirral. The role involves providing commissioning services for individuals with complex care needs eligible for NHS funded care. You will utilise assessment and case management skills, working with patients, families, and carers, and will be responsible for the entire process from referral to assessment, care commissioning, and case management. A valid driving licence and access to a vehicle are essential for travel across the Wirral.
Essential criteria
- Educated to degree level or equivalent level of experience of working in specialist area.
- Evidence of post qualifying and continuing professional development.
- Current NMC Registration.
- Knowledge and Expertise
- Full understanding of Social Services and other Local Authority functions
- Conversant with national and local guidance on continuing care/NHS Funded Care for adults.
- Strong understanding of the operational management and clinical role of the NHS.
- Understanding of the patient experience agenda & its contribution to quality.
- Knowledge & understanding of the current NHS agenda and of primary care.
Desirable criteria
- Understanding of the commissioning agenda including its scope, structure, methods of operation, processes and inter-relationships
- Track record in clinical quality & performance activities.
- Within a Community setting.
- Experience of partnership working with other agencies.
