Role summary
This role involves supporting individuals with mental health difficulties to achieve their recovery goals and live independently within their communities. You will provide practical and emotional support, deliver evidence-based interventions, and work collaboratively with various health and community partners. The position requires building therapeutic relationships and helping people develop confidence and skills for independent living. You will support individuals in their homes, community settings, and during inpatient stays.
Key responsibilities
- Support people with mental health needs to work towards their recovery goals and engage with their care plans.
- Deliver evidence-based interventions under supervision, helping individuals develop coping strategies, build confidence, and improve wellbeing.
- Provide practical and emotional support to individuals and their carers, promoting independence and positive outcomes.
- Help identify changes in a person's mental health and work closely with the wider multidisciplinary team to ensure appropriate support is in place.
- Work collaboratively with health, social care, housing, voluntary sector and community partners to provide joined-up care.
- Contribute to service development, quality improvement initiatives and the ongoing evaluation of the care provided.
Essential criteria
- NVQ Health and Social Care or equivalent, or willing to undertake full training to achieve this standard
- Experience of working with people e.g in a caring situation, dealing with the general public.
- Experience of working in community settings.
- Experience of the assessment process and positive risk management.
- Must be a car driver with a valid driving licence or have access to transport with appropriate business insurance in order to travel throughout the Trust, to meet the needs of the service.
Desirable criteria
- Previous experience in caring for people with a severe mental illness.

This is a varied and rewarding role where no two days are the same.
