Role summary
This is a permanent, part-time Band 5 Community Staff Nurse role within the Out of Hours Community Nursing Service in Halton, covering evening shifts. The post holder will deliver high-quality, person-centred care to housebound patients in their homes, managing a delegated caseload. Responsibilities include wound care, palliative care, catheter care, and long-term condition management. The role requires effective communication, independent work, and collaboration within a supportive team.
Essential criteria
- NMC registered adult nurse
- Evidence of level 5/6 study

- Male & Female catheterisation.
- Supra pubic catheterisation.
- Tissue viability qualification.
- Leg Ulcer management qualification.
- IV Therapy
- Chronic disease management.
- Experience of working within palliative care.

- Teaching & assessing skills
- Physical Skills
- IT literate and able to use standard office software and bespoke systems.
- Physical Effort
- Good physical ability is required to undertake moving and handling tasks within patients homes, using appropriate equipment and adhering to safe moving and handling procedures.
- Previous community experience
- Post-registration nursing experience

Job summary
An exciting permanent, part-time opportunity has arisen for a motivated
and compassionate Band 5 Community Staff Nurse to join our Out of Hours
Community Nursing Service in Halton covering the evening service between 17:00
23:00 3 x shifts a week including Saturdays and Sundays.
This role is ideal for a registered nurse who is enthusiastic about delivering
high-quality, person-centred care to patients in their own home and supporting
people to remain safe and independent within the community.
You will work as part of a supportive and integrated team, caring for housebound
patients with a range of nursing needs including wound care, palliative care,
catheter care and long-term condition management.
We are looking for someone who communicates effectively, works well both
independently and as part of a team and is committed to continuous professional
development. Community nursing is a varied and rewarding area of practice,
