Role summary
This is a part-time, 12-month secondment opportunity for a Registered Nurse to join the Swadlincote Community Nursing Team. The role involves delivering evidence-based care to support patients in their homes, working closely with general practice and therapy services. Responsibilities include assessing, planning, implementing, and evaluating individualised patient care, promoting self-care, and guiding junior staff. The successful candidate will contribute to a holistic integrated service aimed at preventing hospital admissions and supporting safe discharges.

Main duties of the job
