Role summary
This role is for a Registered Adult Nurse to deliver high-quality physical healthcare within the community. You will work autonomously within an Integrated Care Team, managing a defined community caseload and coordinating patient care. The position focuses on preventing hospital admissions, supporting timely discharges, and promoting re-enablement and rehabilitation. - BSc Nursing or RN currently registered with NMC.Practice assessor/supervisor.Post registration, specialist training relevant to speciality.Current (UK/EU) Driving Licence and access to own vehicle for work purposes is required.

- Own transport and driving license, in order to be able to travel

Job summary
We are seeking a Registered Adult Nurse (NMC Adult/Level 1) who is passionate about delivering high-quality physical healthcare within the community. This role is for Adult Registered Nurses and is not suitable for Mental Health or Learning Disability nursing registrations unless applicants also hold current Adult Nursing registration."
The service is seeking a Community Nurse who is keen to work and develop their skills within the community setting.
As a Community Staff Nurse, you will work within the Integrated Care Team, led by the Integrated Senior Clinical Lead Community Lead. You will be working alongside other Health Care Professionals. You will be an autonomous practitioner and have excellent communication and interpersonal skills.
You will work to a high professional standard, which will enable us, as a team, to provide high quality, person-centred care. The post holder will have responsibility for managing co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, Holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.
Please note you will need a full driving license and access to a vehicle for work purposes.
