Role summary
This role is for an enthusiastic Rehabilitation Assistant to join the North Staffordshire Integrated Community Stroke Service (ICSS). The post involves providing specialised patient-centred rehabilitation in community and outpatient settings, focusing on early supported discharge and long-term recovery for stroke patients. The assistant will work as part of a multi-disciplinary team, implementing and evaluating specialist stroke rehabilitation programmes under supervision. The service operates across a 7-day shift pattern within patient home environments around Staffordshire and Stoke-on-Trent.
Key responsibilities
- Perform at a high level of competence within agreed protocols under the supervision of a registered professional.
- Take responsibility for own workload within a broad framework overseen by registered staff to implement, deliver and evaluate specialist stroke rehabilitation programmes.
- Deliver defined therapeutic input independently with access to registered staff for advice and direction as needed.
- Use technical and creative skills in the provision of therapeutic activities for individual patients and/or group sessions within the home environment or an outpatient setting.
- Act as a professional and be competent as appropriate to the requirements of the role within the Integrated Stroke Service.

- Ability to manage time effectively.
- Demonstrates values compatible with MPFT values and behaviours.
- Commitment to working across 7 days to support consistent rehabilitation ethos
- Shows effective communication in a range of environments.
- Able to work in a team effectively
- Level 2 or above qualification (e.g. NVQ III, QCF 2) or equivalent
- GCSE English and Maths grade c / level 4 or above or Functional English and Maths level 2.
Desirable criteria
- Previous experience of working in a community setting/lone working
- Previous experience of working with patients after stroke
- Experience of supporting service users, families and carers in dealing with the impact of compromised functional ability.
- Working in a multidisciplinary team.
- Able to accept feedback and demonstrate awareness of own development needs.
- Evidence of recent CPD activity
- Willingness to study / complete relevant training courses
