Role summary

This role is for an experienced physiotherapist to join a developing stroke service in Carmarthenshire, focusing on acute rehabilitation and early discharge. Based at Glangwili General Hospital, you will support stroke patients to return home and regain independence. The position involves delivering rehabilitation in both hospital and community settings, working closely with acute and community stroke teams. You will also participate in an on-call respiratory service and future 7-day working.

  • Professional registration as recognised by HCPC
  • Post graduate evidence of specialist education.
  • CPD evidence.

  • Post-graduate qualification up to post grad diploma level in relevant specialty
  • Clinical Education Training Course.
  • Member of C.S.P
  • Active participation of specialist interest group.
  • Post-graduate experience in relevant clinical specialty.
  • Completion of junior competency framework.
  • Practical experience to a level that would enable participation on the respiratory on call rota
  • Therapeutic handling and risk assessments
  • Managing a diverse caseload using a clinical reasoning process
  • Inter-professional and team working.
  • Direct involvement with carers and families.
  • Evidence of supporting service compliance with clinical governance requirements.
  • Experience of staff supervision
  • Evidence of student supervision
  • Negoitation of MDT working practices.

Job summary

We are seeking an experienced physiotherapist to join our rapidly developing stroke service in Carmarthenshire, supporting acute rehabilitation and early discharge from hospital.

If you have a passion for stroke rehabilitation working across the stroke pathway this could be the opportunity for you. You will be based at Glangwili General Hospital, Carmarthen supporting people who have predominantly experienced a stroke to return home from our acute stroke units.

The role will aim to deliver rehabilitation in hospital and at home in the local community to support transition from hospital and return to independence. To deliver a person-centred rehabilitation and transition pathway the post holder will work alongside the established acute and community stroke teams (in-reaching and out-reaching) to ensure good continuity of care.
