Role summary
This role is for a Therapy Assistant Practitioner with a special interest and extensive experience in neuro rehabilitation. You will join the Liverpool Community Neuro Rehab Team, a specialist integrated team of Occupational Therapists and Physiotherapists. Key responsibilities
- Assess, plan and implement individualised programmes of care within the skills and competencies expected of the role
- Work collaboratively within the team and with other providers to deliver local and national health priorities and objectives
- Deliver therapeutic interventions in line with national quality and standards and Trust policies.
- Communicate effectively with patients and carers recognising the need for alternative methods of commination to overcome different levels of understanding, cultural background and preferred ways of communication
- Lead on health promotion
- Support clinical staff in managing long term conditions
Essential criteria
- Foundation Degree or equivalent or equivalent work based experience.
- Experience in assessing, planning and implementing individualised programmes of care.
- Experience of previous work in NHS health care.
- Understanding of Clinical Governance and Risk Management.
- Evidence of active involvement in professional development.
- Experience in team working
- Knowledge of Health Promotion strategies.
- Understanding local and national influences on health care delivery, including Public Health
- Excellent communication and interpersonal skills
- Excellent organisational skills
- Risk Assessment skills
- Ability to travel, for work purposes
