Role summary
This role is for a NMC registered nurse to join the Rapid Response team in High Peak and Derbyshire Dales. The team provides high-quality nursing care to support patients in their homes, aiming to prevent hospital admissions and facilitate safe discharges. The successful candidate will work collaboratively with integrated community colleagues to deliver holistic care and promote patient independence.
Key responsibilities
- Provide high-quality nursing care to patients in the local community.
- Assess, plan, implement and evaluate individualised patient care.
- Provide advice and support to junior staff.
- Implement / refer on to relevant services as appropriate.
- Work in close collaboration with Integrated Community Teams/GP surgeries and local acute trusts.
- Respond to priority nursing visits.
Essential criteria
- Full NMC registered Nurse
- Evidence of recent relevant training
- Experience of working in the community
- Evidence of research based/reflective practice
- Knowledge of local and national priorities.
- Understanding of the wider NHS agenda regarding community services
- IT Skills
- Ability to travel across Derbyshire County visiting patients' homes and across the county to access meetings and training
Desirable criteria
- Experience of Community Nursing
- Experience of long term conditions
- Experience of Leg Ulcer management and lower limb assessments
- Experience of end of life care
- Previous experience of using SystmOne
